Merchant Accounts

Application

1. You may be able to apply online INSTEAD of using the following paper form, so be sure to contact a Sales Representative prior to completing this application. (If you have not contacted a Sales Representative yet, please use our Quick Form.)
2. PRINT all pages of the following application.
3. Complete ALL fields.
4. Initial the bottom of each page and sign where indicated.
5. Fax the completed application to 860-298-9617 along with any additional documentation.

 BEFORE YOU START

The merchant application will take approximately 15 minutes to complete. You will need the following:

• URL of your web site (if applicable)
• Voided check (Must match business name)
• 3 months merchant bankcard statements  (if you have processed card sales before)
• Copies of Articles of Incorporation or if partnership, partnership agreement

• 2 years past personal and business tax returns 
  (signed and include all pages)
• Current business financials (signed and dated)

READY TO GO!

Once you have gathered all the necessary information, simply:

1. Submit the setup fee (Online account only) and

2. Download and print the application (pdf)

It only takes a few minutes and you're done!
